Why We Exist

Within a few years, AI will handle a large share of the work that happens on a medical practice's phone lines. Most of healthcare is not ready for that, and most healthcare AI is not built for it. The phone call is still where care often begins, and it still runs on voicemail, callbacks, and notes typed from memory.

Lucea is building the layer that sits inside those conversations. Today we capture practice phone calls and turn them into accurate documentation that staff can trust. From there, we bring licensed clinical protocols into the call, give practice leaders visibility into their operations, and automate more of the work as quality is proven, always with clinicians in control of clinical decisions. We started with pediatric nurse triage, one of the hardest-to-staff jobs in nursing, and we are expanding across practice types and roles.

The early results are strong. A small team has supported more than 25,000 live triage calls and converted every design partner into a paying customer. A triage nurse four days into using Lucea told us: "Don't take this away from me." We are hiring the people who will take it much further.

Why This Role Matters

We're looking for engineers who build at the frontier of AI and hold themselves to the standard of production healthcare software. You spend your own time with the newest models and agentic frameworks because you want to know what's possible next, and you want to ship it into a product clinicians rely on during live patient calls.

We have a product that practices can buy, connect to their phone systems, and use every day, and we're laying the foundation for what comes next: clinical assistance, practice analytics, and voice automation that takes on more of the work as quality is proven. The systems you design now will decide how much better Lucea gets with each new generation of models.

This is a senior, hands-on role. You'll own hard problems end to end, from real-time voice pipelines and phone integrations to note quality and the evals that prove it. You'll use AI to multiply what you ship, help the rest of the team do the same, and raise the quality of everything around you. You'll work closely with our CEO and engineering leadership to shape what we build and how we build it, and as the team grows, your scope can grow with it.

What You'll Own

AI Systems

  • Build and improve the LLM workflows at the core of Lucea: live transcription, note generation, chat, and edge-case handling across real-world call variation.
  • Build the evaluation and testing infrastructure that shows, with evidence, whether outputs are accurate, complete, and consistent, and whether a new model or capability is ready for real practices.
  • Make model choices on evidence: test newer models, use less expensive ones where they meet the quality bar, and cut processing that doesn't add value for users.
  • Build the agentic workflows and internal tooling that multiply what a small team can ship.

Systems and Delivery

  • Own the design and delivery of significant systems across the stack, from ambiguous problem to shipped, reliable software on a growing multi-tenant healthcare platform.
  • Own the systems where correctness and data integrity are non-negotiable, including real-time voice pipelines and integrations with EHRs, telephony, and third-party APIs.
  • Help make phone system integration standardized and repeatable, so each new practice goes live faster than the last.
  • Understand how our users rely on Lucea, find the gaps, and get the fixes prioritized and shipped.

Reliability and Security

  • Raise the bar on reliability: observability, alerting, incident response, and the deployment pipeline.
  • Build security and compliance in by default (HIPAA, encryption, auditability), and help set the standards the team follows.

Technical Leadership

  • Set technical direction in your areas and build alignment across the team.
  • Mentor engineers through code review, design discussion, and pairing, and help level up how the team builds.

What Good Looks Like

  • You bring energy and urgency to the work, and the team does something hard every day.
  • Hard, ambiguous problems become well-architected, reliable systems, because you own them end to end.
  • Each new generation of models makes Lucea better, and adopting them is routine instead of a rebuild.
  • We can show with evidence how good our outputs are, and new capabilities reach customers once they've earned it.
  • The platform scales across more practices and enterprise customers without reliability or security regressions.
  • Other engineers get better and faster because of your reviews, your standards, and the tools and patterns you establish.
  • Our CEO and the team trust you with the technical calls in your areas, freeing others to focus elsewhere.

What We're Looking For

Must Have

  • 7+ years of software engineering experience building and owning production systems.
  • You build with LLMs, not just around them. You've shipped LLM-powered features to production and handled what comes after launch: evals, quality drift, latency, and cost.
  • You live at the frontier. You try new models and agentic tools as they come out, you have a point of view on where they're heading, and you can tell hype from a real step change.
  • AI is how you build. You use AI coding tools and agents daily, and you've built tools or workflows that made the engineers around you faster.
  • You've made architecture decisions that held up under real-world conditions, and you design systems that get better as models improve instead of needing a rebuild with each release.
  • Strong fundamentals across data structures, system design, debugging, and testing, and you can come up to speed quickly in Python and Node.js.
  • You work well with autonomy in a fast-moving, ambiguous environment, and you bring others along with you.
  • You know how to join an existing team and codebase, earn credibility with the engineers who built it, and raise the bar by influence and example rather than title or mandate.

Strong Signals

  • Experience in healthcare, fintech, or other regulated industries, and comfort building for compliance (HIPAA, encryption, audibility).
  • Experience with real-time voice systems: telephony, streaming audio, speech-to-text, or WebSockets.
  • Experience building integrations with EHRs or other complex third-party systems, and making them repeatable.
  • A track record of mentoring engineers or setting technical standards on a team.
  • Things you've built with AI that you can show us: open source, side projects, or experiments.

Who This Role Is Not For

  • You want to move into pure management and stop writing code.
  • You prefer to specialize in a single layer and avoid the rest.
  • You want fully specified tickets instead of owning problems from the ambiguous start.
  • You see AI as a feature for the roadmap, or you're waiting for the tools and best practices to settle before you commit to them.
  • You're excited by the newest model but not by the work of making it reliable for a clinician on a live call.
  • You need a title to have influence.
  • You'd rather rewrite what exists than first understand why it was built that way.

Compensation

  • Base salary: Competitive for a seed-stage health tech company.
  • Equity: early-stage grant. You'll be one of our first senior engineers.
  • Benefits: Medical, dental, vision, commuter, fitness, 401(k), HSA, unlimited PTO.
